On December 14, 2006, the Arizona Department of Economic Security Division of Developmental Disabilities issued a Qualified Vendor Application Approval and Qualified Vendor Agreement Award Notice to Concentric Home Health under Contract Number 1681. This agreement shall begin on December 14, 2006, and shall terminate on June 30, 2009.
